Commitment to the dates given in this application is REQUIRED.

Tuition for the BVYSCO program is free, but guardians agree to pay half of the instrument rental fee monthly.

You will be directed to a new page where you and your child will answer the interview questions via video. Then upload the video of you and your child at the link provided. Child participation is REQUIRED.

We will select applications and select applicants for the program.

Terms of Participation
I understand that the information in this application is true to the best of my knowledge and that the video submitted is our own unedited interview. I also understand that Unity in Strings reserves the right to discharge participants who have submitted false or misleading information in the application process. If accepted as a student of the UIS, I agree to abide by UIS regulations. I agree not to bring any alcoholic beverages or firearms onto UIS premises or to possess or engage in the illegal use of drugs while a student of the UIS. Students are expected (in their academic conduct) to comport themselves with basic standards of honesty and character. Plagiarism, cheating, and dishonesty in UIS- related matters are prohibited and can result in a student’s dismissal from the UIS without refunding any part of the fees paid.
The applicant consents to the creation and distribution of any recording, broadcast, or electronic transmission of any kind, without compensation, made by the UIS or any UIS-approved entities while he/she is a student at the UIS. The applicant similarly consents to the use of his/her likeness in photographs, video, or any visual media created and distributed by the UIS or UIS-approved entities.
Release of Liability
As the legal parent or guardian, I release and hold harmless Unity in Strings, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Unity in Strings its owners and operators or in route to or from any of said premises.
Medical Emergency
The undersigned gives permission to Unity in Strings, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the nearest hospital if The Unity in Strings deems necessary.
Picture Release
The undersigned gives permission to Unity in Strings to use any portraits or video that may include your minor student for promotional purposes.

Parental Consent for Minors Participation in All UIS Events
I hereby give my consent for my son/daughter, to participate in all activities that pertain to the schedule of the Unity in Strings and I release the Unity in Strings (including all directors, officers, employees, agents, or volunteer workers) or event organizers from any claim or liability that may arise directly or indirectly from my child's presence or participation in the activities. I agree that UIS is not responsible for any personal injury, loss or damage that my child may suffer in connection with this event. I also agree to defend, indemnify, and hold harmless the said parties from any claim arising from any wrongful or negligent conduct by my child while a participant at the UIS .
