Total Annual Tuition for this program is $920

Payment options available: choose 8 monthly payments of $115 or 2 semester payments of $460

Terms of Participation
I understand that the information in this application is true to the best of my knowledge and that the video submitted is our own unedited audition video. I also understand that Unity in Strings reserves the right to discharge participants who have submitted false or misleading information in the application process. If accepted as a student of the UIS, I agree to abide by UIS regulations. I agree not to bring any alcoholic beverages or firearms onto UIS premises or to possess or engage in the illegal use of drugs while a student of the UIS. Students are expected (in their academic conduct) to comport themselves with basic standards of honesty and character. Plagiarism, cheating, and dishonesty in UIS- related matters are prohibited and can result in a student’s dismissal from the UIS without refunding any part of the fees paid.
The applicant consents to the creation and distribution of any recording, broadcast, or electronic transmission of any kind, without compensation, made by the UIS or any UIS- approved entities while he/she is a student at the UIS. The applicant similarly consents to the use of his/her likeness in photographs, video, or any visual media created and distributed by the UIS or UIS-approved entities.
Release of Liability
I release and hold harmless Unity in Strings, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of The Unity in Strings its owners and operators or in route to or from any of said premises.
Medical Emergency
The undersigned gives permission to Unity in Strings, its owners and operators to seek medical treatment for the participant in the event the emergency contact cannot be reached. I affirm that the participant is in good physical and mental health, and able to participate fully. I request that our doctor/physician be called and that I be transported to the nearest hospital if the Unity in Strings deems necessary.
Picture Release
The undersigned gives permission to Unity in Strings to use any portraits or video that may include your image and/or likeness for promotional purposes.

Silver Strings Orchestra, please bring $20 for your application fee at orientation on Sept. 28th
APPLICATION PART 2
Thank you for filling out the application form. The next step in the application process is the video audition. A video audition is not required to participate in the Silver Strings Orchestra.
Each participant (adult and child) must submit separate audition videos by uploading a digital file to WeTransfer with their application.
The recording must be a one or two-octave scale and a short piece of your choice. The video can be shorter but must be no longer than 5 minutes. When naming the file, please include your first and last name as well as the instrument you are auditioning on.
Please state the following on the video:
1. Your full name and the parent/ guardian/ child you are participating with. ( My name is... I will be participating with...)
2. Instrument (The instrument I will be auditioning on is the...)
3. The name of the scale and piece you are playing (I am playing a G Major scale and the piece I am playing is Minuet in G by Bach.)
